- Bring the right documents: You’ll need:
- Proof of U.S. citizenship (original + photocopy)
- Valid government-issued photo ID (original + photocopy)
- One recent passport photo that meets all requirements
- Any required companion forms (e.g., DS-64 for lost passports, DS-3053 for minor child consent)
- Do not sign DS-11 ahead of time. Wait until instructed by the passport acceptance agent.
