When to Use Form DS-11: Every Reason You Might Need It for a U.S. Passport

Not sure whether you need to fill out Form DS-11 for your passport application? You’re not alone. This is one of the most common questions we get from travelers, especially those applying for a passport after their passports have expired, or for the first time.

To help you decide if you need to use Form DS-11, I’ve created a list of every situation where the U.S. Department of State requires you to use it.

If any of these scenarios apply to you (or your child), you’ll need to appear in person at a passport acceptance facility near you and submit DS-11.

Expert Tip: You can skip this step by hiring a reputable passport expediting service to submit your application materials on your behalf. They can save you the time and expense of traveling to a Regional Passport Agency on your own.

When You Must Use Form DS-11

Every scenario detailed below requires the passport applicant to use Form DS-11. Please read each situation carefully to determine if any match your situation and passport needs.

Quick Tips for DS-11 Applicants

If you fall into one of the categories detailed above, here’s what to keep in mind:

  • Where to apply: Submit Form DS-11 in person at a passport acceptance facility (e.g., many post offices, clerks of court, or public libraries), a Regional Passport Agency (if you need it fast), or at a U.S. embassy or consulate abroad.
  • Bring the right documents: You’ll need:
  • Do not sign DS-11 ahead of time. Wait until instructed by the passport acceptance agent.
  • Execution fee applies: On top of your application fee, there’s a $35 acceptance fee paid to the facility processing your DS-11. See U.S. Passport Fee Schedule 2025: Routine, Expedited, & More for more details.

Still Not Sure Which Form You Need?

If you’re thinking, “Can’t I just renew my passport by mail?”—you need to be sure you meet all the DS-82 renewal criteria. Otherwise, the State Department will reject your application, wasting your time and possibly delaying your travel plans. When in doubt, play it safe: assume you need DS-11 and schedule that in-person appointment.
